COHO: Choosing Options, Honoring Options provides the following core services:
Annual Public Forums: A series of educational presentations held in the afternoon and evening by medical, palliative, hospice and end-of-life care experts from near and far.
COHO Conversations: Facilitated small group conversations and interactive workshops which encourage meaningful discussion, self-reflection and advance care planning work.
Provider Services: Advance care planning resources, conversation tools and training for physicians, providers and healthcare staff is available. Pilot programs to introduce and implement clinic based quality advance care planning facilitation is offered.
Respecting Choices Advance Care Planning Facilitator Training: COHO instructors offer advance care planning and POLST facilitation courses from the established Respecting Choices program in LaCrosse, Wisconsin.
Speaker Outreach: Community service organizations, faith groups, retirement residences and healthcare providers invite COHO experts to their organization to provide education on many topics related to meaningful end-of-life conversations, advance care planning, palliative and end-of-life care.
